Each carries the version of the matcher that decided it and the footprint you had that day — copied, not referenced, so adding a state next March cannot silently rewrite what we knew this morning.
- The denominator is the point. “We ruled out 205” is noise. “205 of 214 reviewed, and here is why each one” is coverage you can audit.
- It is written before you ask. Recorded at the moment the decision is made, not reconstructed afterwards by re-running today’s logic over last year — which would produce a confident answer about a decision that was never taken.
